What’s Covered Under CDCP?

Note: Coverage is based on CDCP’s national fee guide, which may differ from the Ontario Dental Association (ODA) fee guide followed by Diana Dental. Patients are responsible for any difference.
Coverage Levels Based on Income
Your CDCP coverage depends on your adjusted family net income, calculated using the most recent tax information.

Additional charges may apply if Diana Dental’s fees exceed the CDCP reimbursement rates or if you choose services not covered by the plan.
Understanding the Fee Difference
While the CDCP uses a national fee guide, Diana Dental follows the Ontario Dental Association (ODA) fee guide. This means:
If our fee is higher than the CDCP rate, you’ll be responsible for the difference.
How to Apply for CDCP
Required Information:
- Social Insurance Numbers for all applicants
- Dates of birth and tax filing information
- Adjusted family net income (Line 23600 from your latest Notice of Assessment)
Helpful Tips:
- Only one parent/guardian needs to apply for all children under 18
- Confirm eligibility before applying to avoid delays

Eligibility Criteria
You may be eligible for CDCP if you:
- Are a Canadian resident
- Have no access to private dental insurance
- Have an adjusted family net income under $90,000
- Have filed your most recent taxes
